Certificates

The doctors are able to complete a variety of forms and certificates for registered patients. However, some of these do not fall under the NHS contact and therefore a charge will be applicable. Please read the form carefully before requesting that the doctor completes it. Some require the doctor to have known you for a certain period of time. They will not be able to complete the form if they have not seen you over that period. Even if you have been registered at the practice for that length of time.

Sick Certificates

If you are off work for less than a week, you can collect the form SC2 (Self-Certificate) from reception. You do not need any other certification to return to work.
If your employer insists on a sick note for periods of less than a week, a private certificate may be issued at the discretion of the doctor. The current charge for this is £10.00.
If you are off work for more than a week, you will need to see a doctor to obtain a statutory sick note.


Other Certificates And Claim Forms

Please ask advice from reception if you have a form that you think the doctor needs to fill in. You may be asked to leave it at reception and collect it at a later date. A charge will be made for forms that are not covered by the NHS. Current charges are

£10.00 for a short certificate
£20.00 for a certificate/claim form (such as Health Insurance Claim Form, Travel Insurance Claim form, etc)
£50.00 for a long form/medical report.

Please do not ask the doctor to fill in a form or write a letter during surgery as this causes delays for other patients.


Letters About Housing

Letters regarding housing will not be written except at the formal request of the housing department or housing association. Please do not ask for letters from the doctors as we know from experience that these are a waste of time and will not help your application.


Passport And Driving Licence Applications

The doctors do NOT sign passport application forms or driving licence applications.
