To access GP services you need to be a registered patient of the Practice.
We are accepting new patients onto our list.
You need to be registered before you can make an appointment. If you need an appointment urgently please phone the surgery. Depending on the time of day and appointment availability you may be able to register and book an appointment for the same day.
To register you need to live within our Practice area.

You need to complete the registration form (GMS1) and the New Patient Questionnaire.
These need to be submitted to reception.
You will need to show proof of address when you register.
Once registered (if you are over 16) you will need to book a New Patient Health Check with the Health Care Assistant or Practice Nurse.
This will only take 10 minutes and she will check your general health and take a brief medical history.
